Our Client is pleased to offer Internal Medicine as one of the branches of its Primary Care Team. Our Internal medicine physician and Advance Practice Providers oversee patient care in the clinical setting, in local nursing homes and provide coverage of Palliative Care in the inpatient setting. As a practice, they work closely with our Care Coordination Team on meeting patient’s care needs. In addition, providers are able to assist patients with needs such as DOT and FAA physicals as well as Annual Wellness Visits and Advanced Care Planning. Our providers apply clinical expertise to the diagnosis, treatment, and compassionate care of adults across the spectrum from health to complex illness. They are especially well trained in the diagnosis of puzzling medical problems, in the ongoing care of chronic illnesses, and in caring for patients with more than one disease.
Mix of outpatient and nursing home patient care. Work week is typically 4 days per week, with 3 clinic days and one full day spent in nursing home, making rounds and visits. Additional time charting and evaluation of results is anticipated. It is anticipated the APP will grow to evaluate the complex medical needs of clinic and nursing home patients with minimal oversight by the physician, allowing the physician to expand access in the outpatient clinic setting for additional new and follow up patient visits.
Expectation for APP to grow their patient list while supporting the practice physician and assisting with access for the practice patients with same or next day clinic access. Weekly visits to nursing home patients is incorporated into the weekly clinic schedule. Hospital consults – Internal Medicine utilizes the GPH Hospitalist staff for admissions. Internal Medicine Providers participate in coverage for the Palative care team for inpatient services.
Clinic Hours: 8 a.m. to 5 p.m.
On-Call schedule – expectation of one weekend on call per month for patient and nursing home contact. A total of 10 days on call per month is the maximum scheduled in coordination with other clinic providers. Participate in call coverage plan, scheduling in conjunction with other providers to cover night and weekend calls for patients and nursing home facilities. Ten days of call, including one weekend per month is expected.
Staff available -- Full-time Office Manager, full time RN coordinator, 3 full-time RN, full time LPN, dedicated office assistant for scheduling; care coordination nursing staff assist in clinic weekly.
How long is the wait for an appointment – 10 day wait time for new patients or follow up visits with the physician. Typical wait time for clinic visits with APP staff is 2-3 days for new patient and same day for follow-up and acute visit needs.
Equipment needs – The clinic has access to point of care testing for urine, blood glucose, INR. In addition, in-clinic EKG and bladder scan equipment on hand.
